Care Services

We offer comprehensive support services to people living with HIV disease in Sonoma County. We walk you through the maze of health care services and help you address your health and social service needs. We provide benefits counseling, information and referrals, housing and volunteer services. Our team has extensive experience in HIV care and local resources. All our services are provided free of charge and are completely confidential. 

Information and Referrals

You will work with a Case Manager to help you find and utilize the services you need. We can help you gain access to information and referrals, housing search assistance, emergency financial aid (if eligible), HIV-specific medical care, counseling and support groups and self management.

Benefits Counseling

Face to Face has a benefits counseling specialist who can assist clients through the application process for help in resolving problems they may encounter while applying for various benefits programs. We can also help determine eligibility for benefit programs such as Social Security, MediCal, MediCare, disability, health, and life insurance, and more. We can support clients with the application process and filing claims.

Housing Assistance

We assist eligible clients in obtaining permanent housing and emergency rental assistance.

Volunteer Services

You can request help with getting transportation to and from medical, dental and therapy appointments, in-home practical support, moving, and personal coaching.

Reconnect Desk

Reconnect is a philosophy that supports people with HIV to re-imagine their futures.  It seeks to address social, emotional, educational and vocational needs and to equip individuals to make positive steps to explore new possibilities which lead to satisfying, meaningful and healthy lives.  We encourage you to use our Reconnect Desk, which includes a computer with high-speed internet, printer, FAX, and phone, to search for employment and housing, update your resume, research community resources and access information.  No appointment is necessary; simply sign in with the receptionist.
